How Can a Community Health Worker Help You?

Community Health Workers (CHWs) have formal training and are credentialed in the skill sets needed to provide health services to your clients.  Studies have concluded that:

  • CHWs have been shown to be effective in increasing knowledge and promoting behavior change1;
  • CHW interventions have been shown to improve utilization patterns2; and
  • CHW care management has produced significant reductions in:
    • Emergency room visits
    • Hospital admissions
    • Total patient cost3
